AZ Hourly Rates and Benefits
Pro-Med is a member of the Arizona Hospital and Healthcare Association (AzHHA)
and has been awarded their statewide Travelers Registry Program Contract for
2003-2005
Non-Specialty Hourly Rates
$29.00 per hour and $50.00 per hour for overtime, holidays, and callback time
(includes Couplet Care, Gen Peds, Med/Surg, Med Rehab, Ortho, Post-Partum,
Psych, and Subacute)
 Specialty Hourly Rates
$31.00 per hour and $51.00 per hour for overtime, holidays, and callback time
(includes BMT, Cath Lab, CCU, CVICU, CVOR, Dialysis, ER, ICU, L&D, NICU,
OR, Radiology, and Prog Tele)
Housing Allowance
$1,050.00 per month
Pro-Med Completion Bonus
$500.00 at the end of every 13-week contract

Healthcare PPO Effective the day you begin working with
Pro-Med
Retirement Plan Through Fidelity Investments
For 2004, you may contribute up to $9,000.00 of your total compensation. Pro-Med will match, dollar for dollar, up to 3% of your total income.

Referral Fee
$300.00 when anyone you refer completes his/her first 13-week assignment
